Brief New Patient Paperwork
Prior to your scheduled appointment, please click on the New Patient Paperwork button (located below) to complete and submit your health history and pre-registration paperwork at your own convenience. These forms provide us with the information needed to properly address your health concerns. If you should have any questions regarding the paperwork, please contact our office at (920) 434-2221. If you are unable to complete this paperwork before your scheduled appointment, we ask you arrive a few minutes early so that it is completed before your appointment time.

Insurance Coverage
Howard Chiropractic Clinic contracts with a wide range of insurance companies, however we do not accept the following providers as they are out-of-network.
Medicaid/Forward Health/Badgercare, Molina, Tricare/Champ VA, Security Health Plan, and Together W/CCHP.

Physical Examination
A number of specialized tests will be performed to determine your condition and locate areas of dysfunction. These tests include examining areas of concern, reflexes, muscle strength, joints, spine, neck, hip flexors, range of motion, and posture.
Diagnostic Imaging
Certain issues may require us to take x-rays to assist in developing the most effective treatment plan for you and/or rule out a more serious condition. We will advise you of any diagnostic imaging needed to complete your examination.
Recommendations
After our doctors have thoroughly reviewed your case, they will offer you some recommended choices of care along with a treatment program tailored to your condition.
Same Day Treatment
Patients will generally be provided with their first treatment during this same visit. This may include spinal adjustments, laser therapy, and/or soft tissue therapies.
Home Instructions
Patients will be given home care instructions, including ice or heat application, avoidance of certain activities or positions, as well as home exercises and/or stretches.
Schedule Your Next Appointment
At the end of your initial visit, we will schedule your follow-up appointment per your treatment plan.
